First Year- One Comprehensive Exam

First Year- One Periodic Exam

First Year- Full Set of X-rays

Subsequent Years- Two Periodic Exams

Subsequent Years- 4 Bitewing X-rays

Every Five Years- Full Set of X-rays or

One Emergency Exam

Individual X-rays as needed

2 cleanings (in absence of infection)

2 Fluoride treatments (under age 18)

Two oral cancer screenings

50% off additional cleanings

15% off all treatment (excluding
Cosmetic procedures and Orthodontics)

*$245 a year for each additional family member
